We welcome new patients living at any Coventry address to register with us.
Registering with a GP in England is free, and you don’t need ID, proof of address, immigration status, or an NHS number. GP surgeries are often your first point of contact for health issues—they can treat conditions, give advice, and refer you to other NHS services.
You don’t need ID to register, but the following can help:
- Passport
- Birth certificate
- HC2 certificate
- Rough sleepers' ID badge
- Hostel / Accommodation registration
- Mail forwarding letter
If you’re homeless, you can use a temporary address, like a friend’s, a day centre, or the GP surgery itself.

Please note that all fields are mandatory, including NHS number. If you don’t know this, you can ask your existing GP for it, or you can find it here:
Overseas Visitors
Please note that overseas visitors are entitled to free NHS GP services once they register with the practice. Hospital services apart from A&E may attract a charge.

Accountable GP
You will be allocated a GP on your medical record. This is primarily for admin purposes within the Surgery.
If you would like to know who your accountable GP is please contact us via email at admin.cheylesmoresurgery@nhs.net
This does not mean you have to see/speak to the same GP for your appointments.
