Are you interested in finding out more about our practice and getting involved with the development of health services?

We are happy to accept and consider comments and suggestions from our patients.

Please present your views by:

We always try to offer the best service possible, but there may be times when you feel this has not happened. If you are dissatisfied with any aspect of our service, you may wish to complain.

Please email the Practice Manager at admin.cheylesmoresurgery@nhs.net providing full details and the issue will be thoroughly investigated.

We will acknowledge that we have received your complaint/concern within 3 working days (via automatic email reply).

We will then provide you with a formal response to your complaint/concern within 14 working days.
